Social Worker (Inpatient/Med Surg)

Incumbent is a professional social worker assigned to work at the Fairfield Based Outpatient Clinic assigned to the primary care clinics and to VA inpatients at David Grant Medical Center, Travis Air Force Base as a member of the multidisciplinary treatment teams. The incumbent must use a high level of skill in assessing and treating the basic psychosocial problems of veterans. Referrals typically originate from members of the interdisciplinary team and from veterans and/or their families. The incumbent must complete assessments to determine the presenting problem, to determine in-home factors impacting the problem, and to determine obstacles to treatment and/or discharge and to follow through with recommended treatments. Consultation with members of the multidisciplinary team is provided to coordinate a collaborative effort to meet the agreed upon goals for a veteran's treatment needs and include serving as liaison with community professionals regarding needed services for veterans and/or their families. Services include but are not limited to discharge planning, screening/assessment, treatment planning/goal setting, advocacy, referral to service providers, resource usage/development, coordination of non-institutional and institutional services, crisis intervention, education/health promotion and prevention, and supportive counseling. Employs, establishes, and maintains effective therapeutic relationships with veterans and/or their families who are experiencing medical, psychiatric, emotional, behavioral, and psychosocial problems. Completes thorough psychosocial assessments, including psychosocial diagnosis and high-risk assessments, to determine the psychosocial functioning and needs of veterans and/or their families. The incumbent must be able to utilize this assessment in facilitating the veteran's maximum use of treatment for attainment of the highest level of independence, post-discharge; that is possible and practicable. Actively participates as a member of the interdisciplinary treatment team and actively participates through collaboration with veterans and family as well as interdisciplinary treatment team members in the development and implementation of treatment goals, interventions, and discharge plans Work Schedule:

Must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if degree meets accreditation standards for a master of social work. Must have a current and active license or certification by a state at the advanced practice level which included an ASWB advanced generalist or clinical examination, unless grandfathered by the state which you are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice). Specialized Experience for GS-11:
Requires a minimum of one year of post-MSW degree professional social work experience in the field of health care. This experience must have been in a clinical setting and must have demonstrated the potential to perform advanced assignments independently. (A clinical setting is a medical center, clinic - medical or psychiatric, residential treatment center, or any other type of facility where social work participates in a collaborative treatment and is identified with the medical profession.) Experience must have been obtained AFTER earning a Master's Degree in Social Work. SUBSTITUTING EDUCATION FOR EXPERIENCE AT THE GS-11 LEVEL:
To meet the required specialized experience for the GS-11 level, applicant may substitute a doctoral degree in social work from a school of social work.
