RN-Mental Health Intensive Case Mgt-Rural Access Network Growth Enhancement (MHICM-RANGE)-Fairfield

The responsibility of the MHICM-RANGE RN is to provide and direct the delivery of quality nursing care to a diverse Veteran population, including our rural Veterans. RN has the knowledge & skills necessary to apply aspects of the nursing process within a collaborative, interdisciplinary mental health practice setting. RN serves on a team which provides community based case mgt to Veterans with serious mental illness and a history of high hospital use. RN helps to coordinate plans for nursing interventions and participates in multidisciplinary treatment plans for identification of problem areas and appropriate interventions. RN is responsible for completion of required clinical records as well as forms for evaluation and monitoring of service delivery and outcome RN serves as a primary and secondary case mgr for Veterans and assesses their mental and physical status RN assists Veterans with social/recreational outings in the community RN uses knowledge to effectively implement health promotion and prevention interventions, assists in managing acute/chronic illness states and assists Veterans in attainment of optimal levels of functioning through rehabilitation. RN assists Veterans in the community to maintain or improve their health and mental capabilities by performing functions:
maintaining medication schedules, doing health assessments, monitoring vital signs and assisting in maintaining medication boxes. RN advises the Veteran on functions that are essential to personal safety, self-preservation and comfort in the community. RN assists with research studies in collaboration with other members of the team and applies the findings to practice. She/he serves on Nursing Service Committees and task forces as assigned. RN has knowledge-based case mgt to assigned Veterans while developing the technical competencies required for the specific practice setting. RN has in-depth knowledge regarding Veterans' illness and psychotropic medication, and provides medication education as needed. RN has skills to de-escalate agitated Veterans & to manage crisis situations RN has skills to intervene effectively & therapeutically with Veteran's families RNs assigned to the case mgt program are required to have knowledge of community resources, make field visits to Veterans and must possess a valid driver's license. The incumbent's driving record will be reviewed through the DMV Employer Pull Notice (EPN) program annually to ensure Veteran & staff safety. Work Schedule:

A current, unrestricted RN licensure from any state or territory of the U.S. Graduate from an accredited school of nursing recognized by the VA (Commission on Collegiate Nursing Education (CCNE) or The Accreditation Commission for Education in Nursing (ACEN). Demonstrated abilities in establishment of priorities and time management Excellent interpersonal skills, excellent verbal and written communication skills and the ability to work effectively in a diverse interdisciplinary environment Knowledge of Patient Education (adult learning) principles Preferred Requirements:
Current proficiency in Basic Life Support (BLS) through the American Heart Association (Required prior to assuming clinical duties and must remain current -VHA Directive 11-77) At least two years of RN experiencein Psychiatric nursing within the past five years. National certification or current preparation and qualifications to sit for exam. BSN and/or Master's degree from an accredited school of nursing recognized by the VA (Commission on Collegiate Nursing Education (CCNE) or The Accreditation Commission for Education in Nursing (ACEN). Member of an appropriate professional nursing organization and seeks continuing education and professional development opportunities consistent with the annual learning plan. Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree. Physical Requirement statement:
Use and function of all normal motor and sensory anatomy for provision of patient care, reading, writing, listening, observing/seeing, heavy lifting, bending, pulling, pushing, stooping, squatting, walking, standing, use of fine motor muscles; visual and auditory acuity; emotional intelligence to respond to stressful and/or emergent situations and crises appropriately and calmly; self-care skills to maintain physical, psychological, and emotional health. Examples include:
heavy lifting of 45 pounds and over, heavy carrying of 45 pounds and over, straight pulling, pushing, reaching above shoulder, use of fingers, both hands required, walking, standing, kneeling, repeated bending, ability for rapid mental and muscular coordination simultaneously, near vision correctable at 13 to 16 to Jaeger 1-4, far vision correctable in one eye to 20/20 and to 20/40 in the other.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
